Qualifications
- Current certification as a Board Certified Nurse Practitioner (NP) or Physician Assistant (PA) with a minimum of 4 years of Family/Internal medicine, preventative medicine, and/or functional medicine experience. The candidate will preferably have experience in weight management, telehealth, family medicine, chronic condition management, and remote patient monitoring
- Resides in Eastern, Central, and Mountain time zones.
- Must have licenses: Virginia, Michigan, Texas, Kansas, New York, New Jersey, Florida, South Carolina, North Carolina, Connecticut, Arizona, Ohio, Indiana, California, Tennessee, and Oklahoma
- Strong clinical assessment and diagnostic skills, with the ability to formulate effective treatment plans
- Positive, courteous, and professional communication and interpersonal skills, with the ability to establish rapport and motivate patients remotely
- Proven experience working in a telehealth setting, remote patient monitoring, and a fast-paced environment
- Proficiency in using telehealth platforms and electronic health records (EHR) systems
- Commitment to ongoing professional development and evidence-based practice
- Experience working in a remote-only environment
Responsibilities
- You will provide comprehensive and personalized support to patients seeking assistance with Remote Patient Monitoring (RPM) and Remote Therapeutic Monitoring (RTM) through our telehealth platform
- As a vital member of our team, you will play a pivotal role in promoting healthy lifestyles and assisting patients in achieving their weight loss goals, ensuring long-term success
- Complete synchronous telehealth consultations with patients to assess their medical history, current health status, and weight management goals
- Develop personalized treatment plans tailored to individual patient needs by prescribing medicine when necessary and appropriate, and providing counseling on nutrition, exercise, and other factors influencing weight management
- Monitor patient progress closely, including regular follow-ups, adjustments to treatment plans, and motivational support
- Collaborate with healthcare team members to ensure comprehensive and best-in-class support for patients
- Maintain accurate and detailed electronic health records (EHR) of patient consultations and assessments
- Stay updated on the latest advancements and best practices in chronic conditions, obesity, and weight management, integrating new knowledge into clinical practice
- Work under the guidance and collaboration of the owner of the medical group
Schedule
- Available for coverage from 9/1/26 to 1/31/26, with possibility of extension.
- Hourly wage: $65-$70 per hour
- Schedule: Monday-Friday, 30-40 hours per week
