About the Role

Mend is seeking a dedicated and compassionate Care Navigator (Medical Assistant) to join our team. In this role, you will be responsible for providing remote monitoring services, delivering health coaching support, and facilitating the enrollment process for new patients. You will work closely with our healthcare providers, wellness coaches, and technical team to ensure seamless patient care, education, and engagement.

As a Remote Monitoring Medical Assistant at Mend, you will play a crucial role in helping patients lead healthier lives through personalized wellness support, nutritional guidance, and proactive health management.

What You’ll Do:

• Utilize Mend’s remote monitoring platform to track and monitor patient health data from various medical devices and sensors.
• Review patient metrics, identify significant changes or abnormalities, and alert healthcare providers when intervention is needed.
• Communicate regularly with patients to ensure compliance with monitoring protocols and address questions or concerns.
• Provide health coaching support to help patients adopt sustainable habits in nutrition, physical activity, sleep, and stress management.
• Conduct wellness check-ins to reinforce progress and maintain motivation toward personalized health goals.
• Educate patients on nutrition best practices that support recovery, chronic condition management, and long-term wellness.
• Guide new patients through the enrollment process, including collecting necessary medical information and consent forms.
• Track and document patient progress toward wellness and nutrition milestones within Mend’s digital platform.
• Address patient inquiries, troubleshoot technical issues, and provide ongoing support as needed.
• Participate in team meetings and contribute to the development of remote monitoring and wellness coaching protocols.
• Verify insurance coverage and coordinate billing information when applicable.
• Empower patients through education and motivation, helping them feel confident in their ability to manage their own health.
• Assist in scheduling initial telehealth appointments with healthcare providers.
• Maintain accurate patient documentation and uphold HIPAA compliance at all times.

• Previous experience in a healthcare, wellness, or clinical setting.
• Certification or background in health coaching, nutrition, or wellness counseling

• Understanding of nutritional science and its role in healing, recovery, and chronic disease prevention.
• Empathetic communicator who can coach patients through behavior change and self-management of their health.
• Comfortable in a fast-paced start-up environment where innovation and flexibility are key.
• Highly detail-oriented with excellent organizational and follow-up skills.
• Data-driven mindset with the ability to connect patient outcomes to measurable goals.
• Self-starting, creative problem solver with a collaborative spirit.

Qualifications

• 3+ years Medical Assistant, nationally certified
• Experience with telehealth platforms, remote patient monitoring, or digital health tools a plus.
• Excellent communication skills, both written and verbal.
• Compassionate, patient-centric approach to care.
• Ability to maintain patient confidentiality and adhere to HIPAA regulations.
