As a Patient Enrollment Specialist, your primary responsibility will be to facilitate the enrollment and activation of patients into a comprehensive healthcare program that includes remote patient monitoring, chronic condition management, and medical nutrition therapy. You will play a crucial role in ensuring that eligible patients are identified, educated about the program, and successfully enrolled, while maintaining a high level of patient satisfaction. When needed, you will assist patients with technical issues during enrollment and ongoing use of their devices.
You will:
Patient Identification and Screening:
- Collaborate with healthcare providers, clinics, and hospitals to identify potential candidates for the program.
- Assist mend RNs with thorough screenings to assess patients' eligibility for enrollment based on specific criteria, such as chronic conditions and medical needs.
- Maintain the status spreadsheet of patients identified and progress toward enrollment.
Patient Outreach:
- Establish a process for patient contact from identification to enrollment including multiple modes of communication: SMS, email, phone.
- Manage enrollment goals from baseline to improvement based on outreach plan
Patient Education and Counseling:
- Provide detailed information to eligible patients regarding the program's benefits, services, and expectations.
- Explain the remote patient monitoring process, chronic condition management strategies, and the importance of medical nutrition therapy.
- Address any concerns or questions patients may have, ensuring they have a clear understanding of the program's objectives and requirements.
- Communicate back to the team opportunities to improve communication / marketing materials based on patient response
Enrollment Process Management:
- Guide patients through the enrollment process, including completion of necessary paperwork and consent forms and device activation.
- Ensure accurate and timely documentation of patient information, medical history, and other relevant data.
- Collaborate with healthcare providers, insurance companies, and other stakeholders to obtain necessary authorizations and approvals for enrollment.
Coordination and Communication:
- Maintain regular communication with patients, healthcare providers, and program staff to facilitate smooth enrollment and ongoing participation.
- Coordinate with internal teams to schedule necessary assessments, consultations, and educational sessions.
- When necessary, collaborate with billing to ensure proper insurance coverage and reimbursement processes are followed.
Program Monitoring and Evaluation:
- Assist in monitoring patient engagement and adherence to the program.
- Collect and analyze data related to patient outcomes, satisfaction levels, and program effectiveness.
- Identify areas for improvement and provide feedback to the program management team.

- Familiarity with healthcare systems, insurance processes, and regulatory guidelines.
- Knowledge of chronic conditions, remote patient monitoring, and medical nutrition therapy is a plus.
Qualifications:
- Education in an allied health professional field of study.
- Healthcare-related experience is preferred.
- Strong organizational and time management skills
- Able to work with little supervision while handling numerous projects at once.
- Excellent written/verbal communication skills, especially the ability to communicate telephonically in a professional and effective manner.
- Computer literacy in Microsoft Office and Google Suite
- Ability to handle multiple demands of diverse workload and prioritize urgent issues
- Accountable with strong work ethic
