About the role:

Mend is seeking a dedicated and compassionate Care Navigator (Medical Assistant) to join our team. In this role, you will be responsible for providing remote monitoring services to patients and facilitating the enrollment process for new patients. You will work closely with our healthcare providers and technical team to ensure seamless patient care and monitoring. As a Remote Monitoring Medical Assistant at Mend, you will play a crucial role in delivering our mission and helping patients lead healthier lives.

What you’ll do:

  • Utilize Mend's remote monitoring platform to track and monitor the health status of patients.
  • Review and patient data collected through various medical devices and sensors.
  • Alert healthcare providers to any significant changes or abnormalities in patient data.
  • Communicate with patients to ensure compliance with monitoring protocols and address any concerns or questions.
  • Guide new patients through the enrollment process, including collecting necessary medical information and consent forms.
  • Address patient inquiries, troubleshoot technical issues, and provide support as needed.
  • Document patient interactions and maintain accurate records.
  • Work closely with healthcare providers, nurses, and technical support teams to ensure effective patient care.
  • Participate in team meetings and contribute to the development of remote monitoring protocols.
  • Verify insurance coverage and coordinate billing information as needed.
  • Educate patients on the use of remote monitoring devices and platforms.
  • Assist in scheduling initial telehealth appointments with healthcare providers.
  • Maintain regular contact with patients to ensure they are comfortable with the remote monitoring process.

Qualifications:

  • High school diploma or equivalent; certification as a Medical Assistant.
  • Strong communication skills, both written and verbal.
  • Proficiency in using digital platforms and software.
  • Compassionate and patient-centric approach to care.
  • Ability to maintain patient confidentiality and adhere to HIPAA regulations
